INSTRUCTIONS.

 Right click on the thumbnail image.

Click on "open link in new window".

Once the new window has opened you will see a full sized version of the form.
 
Go to the top tool bar on "File" left click and scroll down to "print"- click

The form will then print.

Do this for all 4 pages.
